3. 4500 Silver Plan
Providence Health also offers an optional dental plan. Additionally, in an effort of convenience, Providence Health offers a singular form for enrollment, change requests, and declination / waiver of coverage.
GNSA's best Oregon health insurance plans from Kaiser Permanente Oregon are:
1. Platinum 500-20 Plan
2. Platinum 0-20 Plan
3. Gold 1500-35 Plan

1. Gold 500 Preferred Plan
2. Gold 1000 Preferred Plan
3. Silver HSA 3500 Preferred Plan
